Whom It May Concern, I am writing to formally express my serious concerns regarding the ongoing lack of communication and professionalism from your office and affiliated practices. Over the past several weeks, I have received appointment reminder messages referencing multiple business names, including “Georgia Pain and Wellness Center, LLC” and “Summit Spine,” without any formal notification or explanation provided to patients regarding a merger, acquisition, practice transition, or name change. As a patient whose personal medical records, health information, and insurance information are being managed by your office, it is extremely concerning that there has been no communication clarifying who is currently responsible for handling patient care and protected health information. At this point, I do not know what the official name of the practice is, who manages patient records, or which entity is responsible for ongoing treatment and communications. Patients should not be left confused about where their private medical information is being stored or who is overseeing their care. Additionally, I have made repeated attempts throughout the month to contact someone regarding an important matter involving clinical notes needed by a medical supply company to complete a prescription for a cane. Despite numerous calls to the phone numbers listed both on Google and within the appointment reminder messages, no one answers the phone, and there appears to be no reliable way to reach office staff, providers, or their assistants. This lack of accessibility is unacceptable, especially when patients are attempting to resolve time-sensitive medical and prescription-related issues. Patients should have a clear and reliable method to contact their providers with questions, concerns, or follow-up requests. Instead, I have spent weeks attempting to get a simple confirmation regarding whether required clinical documentation has been submitted, with no response or assistance from anyone in the office. I am requesting the following: * Clarification regarding the current official name of the practice and the entities involved in patient care and record management. * Confirmation of who is responsible for handling patient medical and insurance information. * Immediate follow-up regarding whether the requested clinical notes have been sent to the medical supply company for the cane prescription. * A reliable method of communication for patients to reach providers or office staff in a timely manner. I hope this matter is taken seriously and addressed promptly, as the current level of communication and patient support has been extremely disappointing and concerning.
